“My legs are swollen” describes a finding, not a diagnosis.
Fluid can accumulate because of a vein problem, impaired lymphatic drainage, medication, infection, injury, or a condition affecting the heart, kidneys, or liver. More than one cause may be present at the same time.
The first useful distinctions are often simple: Did the swelling begin suddenly or gradually? Is it one-sided or present in both legs? Is there pain, warmth, redness, shortness of breath, or a skin wound?
Sudden swelling in one leg deserves prompt attention
A deep vein thrombosis is a clot in a deep vein, usually in the leg or pelvis. It often affects one side and may cause swelling, calf or thigh tenderness, warmth, or red or discolored skin.
Symptoms cannot confirm or exclude a clot by themselves. Risk is higher after recent surgery, hospitalization, major injury, prolonged immobility or travel; during pregnancy or the postpartum period; with estrogen therapy, active cancer, a previous clot, or certain inherited conditions.
New one-sided swelling with pain, warmth, or discoloration warrants prompt medical assessment, particularly when a risk factor is present. Leg ultrasound is the most commonly used imaging test when DVT is suspected. A D-dimer blood test may be useful in selected patients after clinical risk has been assessed; it is not a stand-alone screening test.
Call 911 for sudden unexplained shortness of breath, chest pain—especially pain that worsens with breathing—fainting, coughing blood, or severe lightheadedness. These can be signs of a pulmonary embolism. Do not drive yourself.
Chronic swelling in one leg often points toward a local problem
Swelling that develops gradually in one leg may follow a previous DVT, vein injury, surgery, radiation treatment, infection, trauma, or disruption of lymphatic drainage.
Chronic venous insufficiency occurs when leg veins do not return blood efficiently. The swelling may worsen after prolonged standing and improve somewhat overnight or with elevation. Varicose veins, aching, heaviness, itching, brown discoloration near the ankle, or fragile skin can accompany it.
Lymphedema reflects impaired movement of lymphatic fluid. Swelling may involve the top of the foot and toes and can become firmer over time. Venous and lymphatic problems frequently overlap, especially after years of edema.
A red, hot, painful leg with fever may indicate skin or soft-tissue infection and requires urgent assessment. A new wound, drainage, rapidly spreading redness, or worsening discoloration should not wait for a routine visit.
Swelling in both legs broadens the possibilities
Bilateral swelling is often related to gravity and venous pressure, but systemic and medication-related causes must be considered.
Possible contributors include:
- Chronic venous disease or reduced calf-muscle activity
- Heart failure or elevated pressure on the right side of the heart
- Kidney or liver disease
- Low blood protein from several possible illnesses
- Lymphatic disease
- Sleep apnea and pulmonary hypertension
- Pregnancy
- Prolonged sitting or standing
- Medications
Common medication contributors include certain calcium-channel blockers such as amlodipine, anti-inflammatory medicines, corticosteroids, hormone therapies, gabapentin, and some diabetes or psychiatric medicines. Do not stop a prescription on your own. The timing of swelling relative to a new medication or dose change is useful information for the prescriber.
Swelling from heart failure is more concerning when accompanied by increasing shortness of breath, difficulty lying flat, rapid weight gain, abdominal fullness, or reduced exercise tolerance. Those symptoms deserve timely medical guidance; severe breathlessness, chest pain, fainting, or blue lips requires emergency care.
The examination should guide the testing
A useful evaluation begins before an ultrasound is ordered. The clinician may ask:
- When did the swelling begin, and how quickly did it change?
- Does it improve overnight or with elevation?
- Is there pain, heaviness, itching, numbness, or a wound?
- Have there been previous clots, vein procedures, surgery, cancer treatment, or infection?
- Are shortness of breath, chest symptoms, weight change, or reduced urination present?
- Which prescriptions, over-the-counter medicines, and supplements are being taken?
The examination compares both legs and looks at pulses, temperature, tenderness, varicose veins, skin texture and color, the foot and toes, and any wound. Heart, lung, abdominal, and blood-pressure findings may point toward a systemic cause.
Testing should follow that assessment. A vascular ultrasound can look for acute clot, old venous obstruction, or abnormal backward flow in veins, but the protocol must match the question. Blood tests, urinalysis, an echocardiogram, or other imaging may be appropriate when heart, kidney, liver, thyroid, or lymphatic disease is suspected.
Not every swollen leg needs every test.
Treatment should match the cause
Diuretics can help fluid overload from selected heart, kidney, or liver conditions. They do not correct venous valve failure or lymphedema and can cause dehydration, kidney injury, or electrolyte problems when used without a clear indication.
Compression, walking, calf exercises, elevation, skin care, weight management, medication adjustment, and treatment of venous reflux may each help the right patient. Compression is not automatically safe or sufficient for everyone. Significant arterial disease, acute DVT, skin infection, heart failure status, wound characteristics, and the correct garment strength should be considered first.
The goal is not simply to make the ankle look smaller. It is to identify the mechanism, protect the skin, preserve mobility, and treat any condition that threatens the heart, lungs, or limb.
