Thrombectomy is a procedure that removes a blood clot from a vein or artery. At Flow Heart & Vascular, the relevant outpatient and vascular service scope is selected DVT and limb-artery thrombectomy.

The procedure is not the right treatment for every clot. Anticoagulation is sufficient for many DVTs, and chronic arterial blockages require a different revascularization strategy.

The decision depends on where the clot is located, how long it has been present, symptom severity, limb threat, bleeding risk, underlying anatomy, and the chance that removing it will improve the outcome.

When may DVT thrombectomy be considered?

Selected patients with extensive, recent iliofemoral DVT and significant swelling or pain may benefit from catheter-based clot removal. The goal is to restore outflow and, in appropriate patients, reduce severe symptoms and long-term venous damage.

A serious outflow problem such as iliac vein compression may be treated after clot removal when it is clinically significant. Anticoagulation remains part of care unless a contraindication changes the plan.

Small distal clots and many femoropopliteal DVTs are managed without thrombectomy. More treatment is not automatically better.

When may arterial thrombectomy be needed?

A sudden clot in a limb artery can cause acute ischemia: abrupt pain, pallor, coldness, numbness, weakness, or loss of pulse. This is an emergency that requires immediate hospital evaluation.

Catheter thrombectomy, clot-dissolving medication, angioplasty, stenting, or surgery may be used depending on limb threat, clot location, cause, and available expertise.

How is catheter thrombectomy performed?

After local anesthetic and medication for comfort, a small sheath is placed in a vein or artery. Imaging maps the clot and surrounding vessels.

A specialized catheter removes or disrupts clot by aspiration or mechanical action. Venography, angiography, and intravascular ultrasound may guide treatment and identify an underlying narrowing.

The procedural setting is selected based on urgency, clot location, complexity, other illnesses, and need for hospital resources or surgical backup.

What are the potential benefits and risks?

Potential benefit includes faster restoration of blood flow, reduction of severe swelling or ischemic pain, and protection of a threatened limb in the right setting.

Risks include bleeding, vessel injury, clot migration, recurrent clot, contrast reaction, kidney injury, infection, need for additional intervention or surgery, and failure to restore durable flow.

The balance differs for a stable patient with leg swelling and a patient with a threatened limb. Dr. Patel discusses the specific goal and alternatives rather than presenting thrombectomy as a universal solution.

What happens after clot removal?

Monitoring focuses on the access site, circulation, swelling, pain, and bleeding. Medication commonly continues to prevent recurrent clot, but the regimen depends on whether the problem was venous or arterial.

Follow-up may include ultrasound, evaluation for May-Thurner syndrome or another anatomic cause, compression guidance, and review of provoking factors.

New severe pain, coldness, numbness, weakness, uncontrolled bleeding, chest symptoms, or sudden breathlessness requires emergency attention.

Frequently asked questions

Does thrombectomy replace blood thinners for DVT?

Usually not. Anticoagulation commonly continues because removing visible clot does not eliminate the tendency for new clot to form.

Can an old clot be removed?

Chronic clot becomes organized and incorporated into the vein or artery wall, making removal less effective. Timing and imaging help define realistic options.

Will swelling disappear immediately after DVT thrombectomy?

It may improve quickly, but residual inflammation, valve damage, or lymphedema can prolong swelling. Follow-up remains important.
