What to bring

  • Photo identification and your current insurance card
  • A current medication list or clear label photos, if not already sent
  • Relevant prior results or records that you have not already shared
  • Your intake packet, if you have not already submitted it securely
  • Questions or concerns you would like to discuss

Please arrive 15 minutes early to allow time for check-in.

What to expect during your visit

Your concerns, previous care, and goals guide the visit.

  1. Share your main concern. Describe what has changed, when it happens, and any test result or treatment recommendation you want to understand.
  2. Review your history and results. Dr. Patel considers your symptoms, medical history, medications, and prior records. Let the office know about recent tests.
  3. Agree on next steps. Discuss the findings, whether testing would help, and how the plan fits your priorities. Clarify how you will receive results, whom to contact, and when to follow up.

If a test is recommended, confirm its location, preparation instructions, and appointment arrangements with the office.

LESS PAPERWORK BEFORE YOUR VISIT

Forms and Secure Upload

Send what you already have; fill in only the gaps. Share forms, relevant records, or your medication list without rewriting the same information.

Opens in Hushmail. No Healow account is needed. Up to 20 files and 75 MB total per submission.

  1. New-patient visit: Complete the intake packet’s registration, reason-for-visit, and consent/signature sections. Save the filled PDF, or print and complete it by hand.
  2. Share relevant information: Upload recent heart or vascular test reports, relevant blood work, and your medication list or clear label photos. Skip history and medication sections already covered in your records. Add your main concern, any medication or allergy changes, and your primary care doctor if known.
  3. Need us to request records? Choose “Please request records for me” in the secure form and use the facility checkboxes to point us to relevant testing. Add the location and approximate dates if known, then follow the records-authorization link. Include older tests only when they relate to your current concern or our team requests them.

Already a patient?

Use the same upload link for updated records, labs, or medication information. Complete another intake packet only if the office asks.

Cards and paper copies

You may upload both sides of your insurance card and, optionally, the front of your photo ID. You can bring them and your paperwork instead. If you arrive without completed forms, paper forms are available at the office. Call 480-571-1863 for help or mailing instructions; keep your originals.

For medical information, use the secure upload. Do not send it through ordinary email or the appointment-request form. Please omit Social Security numbers and payment-card details. Uploading documents does not schedule an appointment.

Insurance and Billing

Participation depends on your exact plan, product, medical group, network, referral requirements, and authorization rules. A plan listed by the practice does not guarantee that a particular visit, test, or procedure is covered.

Review the current insurance guidance, then confirm participation, benefits, and expected out-of-pocket costs with both the office and your insurer before your visit.

Getting Your Test Instructions

Preparation varies by test. Follow the specific instructions provided by the office rather than relying on general online advice.

If you have not received instructions, are unsure which test is scheduled, or have a question about the directions you received, call the office before the appointment.
