How can I confirm whether my exact plan participates?
Insurance companies often offer multiple products and networks under the same brand. Call 480-571-1863 with the information on your card. The office will review whether Dr. Patel, the Flow Heart & Vascular group, and the current office location participate with your specific plan. Your insurer determines final coverage and payment.
Do I need a referral or prior authorization?
Some HMO, Medicare Advantage, AHCCCS, TRICARE, VA Community Care, and delegated medical-group plans require a referral or prior authorization. The office will review what is needed before the appointment.
Do you participate with Original Medicare and Medicare Advantage?
Flow Heart & Vascular participates with Original Medicare and multiple Medicare Advantage plan families. Medicare Advantage participation is specific to the exact plan and network.
Do you participate with AHCCCS?
The current public list includes Arizona Complete Health Complete Care Plan, Arizona Complete Health ALTCS, and AZ Blue Health Choice Arizona. This does not mean that every AHCCCS plan is included; please call to verify your exact plan.
What if my insurance is not listed?
Please call the office. Some employer plans use a third-party administrator or network name—such as UMR, Meritain Health, AmeriBen, MultiPlan, or PHCS—that may differ from the insurance company shown on the card.
Does being in network mean every service is covered?
Not necessarily. Coverage, deductibles, copays, coinsurance, referrals, and prior authorization depend on the specific plan and service. The office can help review benefits, but the insurance company determines final coverage and payment.
