New patient forms
Complete the registration, reason-for-visit, and consent/signature sections of the intake packet. Download the packet to your device before filling it in, or print and complete it by hand.

Filling out the PDF on a phone or computer
- Save the packet to your device and open the saved copy in a PDF app that supports fillable forms.
- Complete the form, save it, then close and reopen it to check that your answers were saved.
- Upload the completed copy using the secure upload link on this page.
If typing or saving does not work, you can print the packet or complete paper forms at the office.
Downloading or filling in the packet does not send it to our office.
Send forms and records to Flow
Use our secure Hushmail upload for completed forms, relevant test reports, labs, and your medication list. No Healow account is needed.

What to send
- Recent heart or vascular test reports and relevant blood work you already have.
- A current medication list or clear photos of the labels.
- Your main concern and any medication or allergy changes not already in your records.
For an existing patient, use the same upload link for updated records. Complete a new intake packet only if our office asks.
Ask Flow to obtain outside records
Choose “Please request records for me” in the secure upload form, identify the facility and approximate test dates, and follow its authorization link. You can also complete a secure records release authorization.Request your records from Flow
Need a copy of your Flow records, or want us to send them to another clinician? Open our secure message form and choose Medical records request.
Tell us which records you need and whether they are for you or another office. Our team can help clarify the request and any authorization needed.
Request records from Flow ↗You may bring your insurance card, photo ID, and paperwork to the office instead. Paper forms are available. Please omit Social Security numbers and payment card details from uploads.
